The cardiac rehabilitation program is an interdisciplinary behavior change, self-management program provided to patients who have had a cardiovascular event or procedure. Focus of the program is to provide support and education of lifestyle management as it relates to cardiac health and wellness.

Referrals for this program primarily come from local Cardiologists, but also from family physicians and nurse practitioners.  Patients attend the program once per week for 26 visits (six (6) months). Weekly visits provide education on a variety of heart healthy topics, goal setting and individualized exercise prescription.  A more detailed educational program from an interdisciplinary team is also provided each month which include sessions on heart physiology, cardiovascular risk factors, cardiac tests and procedures, cardiac medications, stress management, nutrition, smoking cessation and advance care planning.
