Geriatric Assessment Intervention Network (GAIN)

As a demonstration of the commitment to the expansion of specialized geriatric services with a goal to support seniors across the region, the CE LHIN has supported the creation of specialized geriatric clinics.

PRHC, along with similar clinics at Lakeridge Health, The Scarborough Hospital, and the Rouge Valley Health System, have opened urgent/emergent clinics which will provide comprehensive assessments for frail seniors in the area.

Target Population
Geriatric Assessment Intervention Network (GAIN) Clinics are designed to provide an inter-professional approach to assessment and care for high-risk seniors, generally aged 75+, who:

  • live at home or in a retirement residence*; or
  • have multiple complex medical, functional and psychosocial problems, or
  • have recently experienced a functional or cognitive decline, or
  • are at risk for falling, or have experienced falls

*Seniors living in Long-Term Care Facilities will not be seen in the GAIN Clinic

What patients can expect
Seniors will be assessed and treated as needed. Recommendations and treatment plans will be created and shared with the senior and primary care provider. It is important for seniors to come prepared for their appointment. Please ensure the following items are brought to the clinic:

  • walking aids,
  • eye glasses,
  • hearing aids, and
  • ALL medications, including over-the-counter products (please bring all medication containers, bottles, eye drops, creams, etc)
  • a light lunch/snack - the assessment may take 2 to 3 hours, if tolerated.

The inter-professional team at the GAIN clinic includes:

  • Nurse Practitioners
  • Physiotherapist
  • Occupational Therapist
  • Social Worker
  • Pharmacist
  • Clerical Support
  • CE CCAC Health Career Case Managers
  • Geriatrician or physician who specializes in geriatrics available for consultation when required.

Access to the GAIN Clinic

  • The team works collaboratively with the Geriatric Emergency Management (GEM) Nurses and with other professionals within the Emergency Department (ED) to identify appropriate seniors who will benefit from the services provided at the clinic.
  • Other seniors who may benefit from the clinic services could be identified during hospitalization and would be referred to the clinic on discharge from hospital
  • Primary Care Providers (Physicians and Nurse Practitioners) will also identify seniors who could benefit from the specialized clinic services and potentially avoid a visit to the Emergency Department

Referrals to the GAIN Clinic
Referrals must be signed by a Physician or Nurse Practitioner (MD/NP). Completed referral forms can be faxed directly to the Peterborough Regional Health Centre at 705-876-5058.
