Adult Diabetes Centre

With awareness and information, the risks associated with diabetes can be reduced. Diabetes education is the key to giving people with diabetes (and their families), the tools to effectively managing their disease.

Our Adult Diabetes Centre provides support, education and care for individuals over the aged of 18 (and their family members), who are:

  • Adults with pre-diabetes
  • Adults with Type 1 or Type 2 diabetes 
  • Newly diagnosed or those interested in updating their knowledge
  • Women with diabetes in pregnancy (Gestational Diabetes)
  • Insulin initiation (physician's request)
  • Insulin Pump Therapy
  • Continuous Blood Glucose Monitoring
  • Community Outreach and Education

Services provided include: 

  • Individual instruction, (one-to-one basis with a Registered Nurse/Registered Dietitian)
  • Group Sessions (allows the client the opportunity to learn from and with others, who also have diabetes)
  • Instruction for clients diagnosed with Impaired Fasting Glucose (IGF) or Impaired Glucose Tolerance (IGT)
  • Initiation of insulin, (includes preparation and administration using insulin pens, pumps and conventional syringes, along with prevention and management of hypoglycemia)
  • Gestational Counseling (information given for diabetes during pregnancy, which includes meal planning, blood sugar monitoring, insulin initiation as directed by physician, and follow up .)

Healthcare Team
Our Registered Nurses and Registered Dietitians who have been certified as Diabetes Educators by the Canadian Diabetes Education Certification Board. They work as members of the inter-professional healthcare team, along with physicians to offer you develop the "tools", (e.g., Advice regarding meal planning, exercise, blood sugar monitoring and use of oral medication/insulin), you need to self-manage your diabetes.
