Nurse Practitioner, Centre for Complex Diabetes Care

Purpose
The Clinical Care Coordinator for the Centre for Complex Diabetes Care (CCDC), a licensed Nurse Practitioner will report to the Manager, Regional Renal and Metabolic Programs and regionally to the Central East LHIN Regional Clinic Manager; working within an interdisciplinary team the Coordinator will provide comprehensive health care for adult clients with complex diabetes conditions. This position will practice at a high level of autonomy and will formulate clinical decisions and development of adult client care plans. The incumbent will be the key liaison with primary care, referral partners and the community to ensure seamless transitions, in and out of the CCDC/Nephrology ambulatory clinics, facilitating collaboration among all partners involved in the ongoing care of the client.

EDUCATION AND EXPERIENCE
• Current RN (EC) registration with the Ontario College of Nurses, with a Nurse Practitioner certificate in Acute Care or Primary Care;
• Recent relevant clinical nursing experience;
• Strong knowledge of, and experience in the care of patients with complex diabetes needs and associated comorbid conditions essential;
• Knowledge of health care system, including service delivery programs for people with diabetes;
• Demonstrated experience in adult education, health coaching, client self-management and case management skills;

SKILLS AND ABILITIES
• Demonstrated knowledge of, and commitment to chronic disease prevention and management;
• Demonstrated exceptional interpersonal, verbal and written communication skills and judgement when relating to clients, families, co-workers and members of the interdisciplinary team;
• Demonstrated ability to build and maintain effective relationships with a diverse range of people, and work effectively within an interprofessional team;
• Demonstrated ability to organize workload and establish priorities effectively;
• Demonstrated problem-solving skills and ability to exercise independent judgement and to assess situations and address problems efficiently and effectively; and
• Motivated self-directed learner, who take initiative and seeks learning opportunities to enhance client care and professional competence.
• Computer literacy, including knowledge of Meditech and computer programs such as Microsoft Office and Internet Explorer an asset;
• Experience within a clinic-based setting an asset;
• A second language an asset.
