<p class=MsoNormal style="margin:0in 0in 0pt;text-align:justify"><span style="font-size:10pt;font-family:Arial">As a member of the GAIN Geriatric Clinic and as a part of the broader Geriatric Assessment Intervention Network, the Registered Nurse, Extended Class, RN(EC) will be primarily responsible for clients referred to the GAIN Clinic. The patient population will be elderly or frail clients who have been identified through the Emergency Department or as direct referrals from the community by community family physicians. Referrals may also come from inpatients that are deemed ALC within the hospital and waiting for a comprehensive geriatric assessment to ascertain discharge disposition. This person will participate in an interdisciplinary team environment. Responsibilities may also include an opportunity to practice in other areas that may be of interest including working with admitted patients within the 4 bed Acute Care of the Elderly (ACE) unit under the direction of the geriatrician and/ or hospitalist. </span></p>

The RN(EC) functions in an expanded role demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and promote wellness. The RN(EC) functions in a collegial and professional relationship with physicians, nurses and other interdisciplinary team members who influence the health environment. The collaborative practice of the RN(EC) includes accepting referrals from, consulting with, or referring to other health care providers in order to promote comprehensive and continuity of care in both primary and acute care specialty practice.</span></p></div></div></div>
